Validez y reproducibilidad de la edad biológica estimada con un algoritmo clínico de reserva funcional comparada con un método basado en biomarcadores

Introducción: La medición de la edad biológica permite cuantificar el proceso de envejecimiento. Recientemente, desarrollamos un algoritmo clínico para estimar la edad biológica a partir de indicadores de la reserva funcional que integran mediciones de función vascular, metabólica, de condición física y valoración del bienestar subjetivo; no obstante, desconocemos su validez y reproducibilidad. Objetivo: Evaluar la validez y reproducibilidad de la edad biológica estimada con este algoritmo de reserva funcional comparada con el método de Klemera-Doubal, ya validado con biomarcadores. Metodología: Estudio transversal que incluyó adultos con edades entre 30 y 60 años de ambos sexos, de quienes se obtuvieron variables sociodemográficas, antecedentes personales, medidas antropométricas, signos vitales, pruebas músculo-esqueléticas, percepciones subjetivas de bienestar, valoración periodontal y paraclínicos para cuantificar la edad biológica. Se utilizó el coeficiente de correlación intraclase (CCI) de acuerdo absoluto, la diferencia promedio con sus respectivos intervalos de confianza al 95 % y el gráfico de Bland Altman como pruebas de validez y reproducibilidad del algoritmo de reserva funcional con relación al método Klemera-Doubal. Resultados: Se incluyeron 125 personas con un promedio de edad cronológica de 45,1±9,4 años y de edad biológica de 47,1±10 años; 46,4% fueron mujeres. La diferencia entre la edad biológica por el algoritmo clínico y el método Klemera-Doubal fue de 2,0±3,6 años; CCI de acuerdo absoluto de 0,953. La reproducibilidad intra-observador (diferencia 0,5±1,4 años) e inter-observador (diferencia 0,4±2,0 años) del algoritmo de reserva funcional fue muy buena; CCI de acuerdo absoluto de 0,988 y 0,977, respectivamente. Los gráficos Bland-Altman mostraron pocas mediciones fuera de los límites del acuerdo sin tendencias evidentes. Conclusión: El algoritmo clínico desarrollado basado en la reserva funcional es un método válido y reproducible para evaluar el envejecimiento biológico; por tal razón, se recomienda su uso en el contexto clínico.
