La calorimetría indirecta. Herramienta para evaluar necesidades energéticas de pacientes con riesgo nutricional

Resumen

La evaluación de los requerimientos energéticos sigue siendo un desafío en la práctica clínica. Hace más de un siglo, Harris y Benedict propusieron una ecuación basada en el peso, la altura, el sexo y la edad en voluntarios sanos. Desde entonces, se han publicado cientos de ecuaciones predictivas para ayudar a los profesionales de la salud a determinar los objetivos de las necesidades calóricas al lado de la cama del paciente. Sin embargo, cuando se comparan las mediciones con el estándar de oro, se ha encontrado que la mayoría de estas ecuaciones son inexactas. La calorimetría indirecta se ha convertido en una herramienta importante para determinar el gasto energético en reposo en diversas condiciones como obesidad, insuficiencia renal, cáncer, fragilidad, COVID 19 y cuidado crítico. Además, esta cuantificación se ha sumado a la evaluación pronóstica y la severidad de la enfermedad. La calorimetría indirecta se está convirtiendo en un recurso de atención para el clínico en su práctica diaria.
